Fertility preservation can give 2SLGBTQI+ individuals more flexibility when planning for the future. Whether someone is considering gender-affirming medical treatment, delaying parenthood, focusing on career goals, or simply wants to keep future options open, freezing eggs, sperm, or embryos can be an important part of family planning.
For trans and gender-diverse patients, fertility preservation is often considered before starting hormone therapy or gender-affirming surgery, as some treatments may impact future fertility. Egg freezing involves stimulating the ovaries and retrieving eggs to be frozen for future use, while sperm freezing is a relatively quick process that allows sperm samples to be stored long-term. Some patients may also choose embryo freezing if they already have a partner or donor they plan to use in the future.
When Should Someone Consider Fertility Preservation?
There is no “perfect” time to explore fertility preservation, but learning about options earlier can provide more flexibility later on. Some individuals choose to pursue fertility preservation before beginning medical transition, while others may explore it while delaying parenthood for personal, relationship, financial, or career reasons.
Because fertility naturally changes with age, especially egg quality and quantity, some patients prefer to preserve reproductive options earlier even if they are not planning to start a family anytime soon. A fertility consultation can help patients better understand timelines, testing, and what options may be most appropriate for their goals.
What Does the Process Look Like?
The fertility preservation process depends on the treatment being pursued.
Egg freezing typically includes:
- Fertility testing and ultrasounds
- Hormone medications to stimulate the ovaries
- Egg retrieval procedure
- Long-term freezing and storage
Sperm freezing is generally a simpler and faster process involving semen analysis, sample collection, and cryopreservation.
For some patients, embryo freezing may also be an option. This involves fertilizing eggs with sperm before freezing embryos for future use. Throughout the process, individualized and affirming care matters. Fertility treatment can feel vulnerable, and patients deserve supportive healthcare environments where they feel comfortable asking questions and discussing their goals openly.
Please note that if eggs, embryos, or sperm are going to be used with surrogacy, additional steps and screening will need to be completed prior to freezing for Health Canada regulations.
